Authorization For Dentistry

Pets must be current on vaccinations and tested according to hospital policy.

Factors that limit our availabilty to detect every dental problem your pet may have prior to anesthesia include:

1. Lack of patient cooperation that impairs proper visualization, especially of back teeth.

2. Periodontal problems that can be detected only by probing under the gum with an instrument.

3. Dental plaque (tartar) that can hide underlying cavities or fractures.

Please be aware that all surgical procedures require an ECG and bloodwork to ensure the well being of your pet.

We authorize such procedures to be performed and agree to pay for services rendered. The nature of the procedures has been described to my satisfaction and I realize that no guarantee or warranty can be ethicallyor professionally made regarding the results or cure. I understand that I assume financial responsiblity for all services rendered and that payment is due at the time the animal is released.

For the comfort of your pet, post operative pain treatment will be administered if extractions are performed. For dogs and cats, this may include an injection and oral medication. The cost for pain treatment is $42.
